Want to save time? We encourage all new patients to fill out the new patient form before visiting our office. It shortens your first appointment, and it lets us review your history in advance so we can talk through a personalized treatment plan while you are here rather than afterwards.

- A photo ID and your dental insurance card, if you have coverage.
- A list of any medications or supplements you take, including dosages.
- Recent X-rays or records from a previous dentist, if you have them.
- Any questions you want to ask — the first visit is the time for them.
